Geographic Limitations: Services are offered exclusively to individuals who are physically located within the states of Arizona, North Carolina, Montana, or Idaho at the time the telehealth encounter occurs. Accessing the Website from other states or countries does not constitute an offer of clinical care in those jurisdictions.

Idaho Telehealth Access Act Compliance: Pursuant to Idaho Code § 54-5701 et seq., care in Idaho is delivered by licensed providers (e.g., Susan Anderson, PA-C) operating under collaborative practice agreements with licensed collaborating physicians (e.g., Dr. Fadul, MD). Upon your written request, treatment summaries will be transmitted to your designated primary care provider.
  4. No Session Recording: Neither the patient nor the provider may record (audio or video) any telehealth encounter without explicit, prior written consent from both parties.
  1. PATIENT RESPONSIBILITIES & USER CONDUCT
  1. Accurate Information: You agree to provide complete, truthful, and accurate medical history, medication lists, allergy details, and identity disclosures. Providing falsified, misleading, or incomplete information constitutes grounds for immediate termination of care.
  2. Prescription Safety: Medications prescribed by The Healthy Place PLLC are for your sole, personal use. You agree not to transfer, sell, or share prescribed medications or needles with any third party.
  3. System Security: You agree not to attempt to gain unauthorized access to any portion of the Website, patient portals, or connected IT infrastructure, or transmit malicious code, viruses, or spam.
  1. PRESCRIPTIONS & COMPOUNDED PHARMACEUTICALS
  1. Clinical Discretion: The issuance of any prescription is at the sole professional medical judgment of your provider following a clinical evaluation. Booking an appointment or paying a consultation fee does not guarantee a prescription.
  2. Compounded Medications: You acknowledge that compounded formulations (e.g., compounded Semaglutide or Tirzepatide) are customized preparations created by US-licensed, accredited compounding pharmacies and are not FDA-approved brand-name products.
  3. Pharmacy Selection: While you maintain the right to choose your preferred pharmacy, prices, shipping times, and availability may vary. The Healthy Place PLLC reserves the right to adjust compounding pharmacy fulfillment partners to maintain product availability and safety standards.
  1. USER ACCOUNTS & PATIENT PORTALS
  1. Account Credentials: To access clinical services, you must create a patient portal account (e.g., via Charm EHR). You are solely responsible for maintaining the confidentiality of your username, password, and access codes.
  2. Unauthorized Access: You agree to notify us immediately at Info@TheHealthyPlacemedical.com of any unauthorized use or security breach of your account.
  1. FINANCIAL POLICY, PAYMENTS & REFUNDS
  1. Direct-Pay Practice: The Healthy Place PLLC operates on a direct-pay, cash-based model and does not accept, bill, or participate in any commercial health insurance plans, Medicare, or Medicaid. You are solely financially responsible for all fees incurred.
  2. Payment Authorization: Payment for clinical services, consultations, lab evaluations, and medication orders is due in full at the time service is scheduled or ordered. You explicitly authorize The Healthy Place PLLC to charge your payment card on file for agreed-upon fees.
  3. Strict No-Refund Policy: Due to the custom nature of medical chart reviews, provider consultation time, and pharmaceutical safety regulations, all payments made to The Healthy Place PLLC are strictly non-refundable once services are rendered or prescription orders are submitted to a pharmacy.
  1. TERMINATION OF CARE & ACCOUNT SUSPENSION

We reserve the right to suspend or terminate your patient account, refuse service, or terminate the provider-patient relationship at our sole professional discretion, without prior notice, for reasons including but not limited to:

  • Breach of these Terms or practice consent forms.
  • Non-payment of fees or failure to maintain an active payment card on file.
  • Verbal abuse, harassment, or disruptive behavior directed toward providers or staff.
  • Falsification of medical information or illegal diversion of prescribed medications.
